How about using a statement like the one below

     SELECT

        CASE
           WHEN position('cessation' in sh.tobacco_comments) > 0 THEN 'CESS1'

           WHEN POSITION ('cess' in sh.tobacco_comments) > 0 THEN 'CESS2'

        END
